if they are taking new patients. 

Call Member Services of your health plan to

be assigned to the provider or clinic.

set up a new patient appointment

*** IF you have Straight (Fee For Service) Medi-Cal, Original Medicare, or Private Insurance with a PPO, you don't need to call your health plan, and can establish care with a provider or clinic directly.

Medi-Cal

Health care for people with low incomes who qualify. Medicaid is paid for by the federal government, but each state runs its own program. In California, Medicaid is called Medi-Cal

 

Fee For Service/Strait Medi-Cal: Traditional Medi-Cal coverage that is not managed by a health care plan. Medi-Cal contracts out directly with the provider/clinic. Do not need a referral from a primary doctor for specialty care, and can be assigned to a primary provider without going through a health plan. Regulated by the Department of Health Care Services. Most people do not have Fee For Service Medi-Cal anymore, and the few that do were enrolled into this type of medi-cal through a traditional medi-cal program.

 

Managed Care Medi-Cal:

Medi-Cal that is managed by a health care plan. Most people in California have a health plan through Medi-Cal. With this kind of Medi-Cal, you will need to go through your health plan to be assigned to a primary care provider, as well as get a referral from your primary care provider for any specialty care. Regulated by both the Department of Health Care Services and the Department of Managed Health Care. there are more rights and protections under managed care plans. 

 

Medicare

Medicare is federal health insurance for: 

  • People who are 65 or older (with 40 quarters of work, which is about 10 years of paying taxes towards medicare)

  • people with disabilities (Must be on SSDI for at least 2 years to be eligible)

 

Original Medicare:

Medicare that doesn't have a health plan managing its benefits.  you can go to any doctor or facility that accepts original medicare.  

 

Medicare Advantage:

Medicare with a health plan, also called a medicare advantage plan. Your health plan manages your medicare benefits. Similar to Managed care medi-cal, your health plan will outline the network of care that you have access to. As will Medi-Cal you will need a referral for specialty care from your primary doctor. 

 

Private Insurance:

Private Insurance Companies that are not run by the state or federal government. 

 

You can access private insurance:

  •  directly through the insurance company

  • Through employment

  • Through your state's healthcare exchange that was created through the affordable care act. Some states have their own state exchange and some use the federal exchange.  Covered California is California’s own exchange program. The exchange enables eligible individuals and small businesses to purchase private health insurance coverage at federally subsidized rates. 

 

Covered California:

Private insurance that is purchased through California’s Federal Exchange program, and offers subsidies to those that qualify based on various criteria (including income).

 

Private Insurance with an HMO:

A type of managed care insurance plan that has a clearly defined network of care for you to access your primary care needs. With an HMO you have to be assigned to your primary care physician by your health plan. For any specialty services you will need to get a referral from your primary care physician. 

 

Private Insurance with a PPO:

A type of managed care insurance plan that has a network of care like an HMO, but you are able to go outside of this network for covered services. However you will pay more. With a PPO plan you do not need to be assigned to a Primary Care Physician through the insurance plan, and you don't need a referral to see a specialist. 

 

Private Insurance with an EPO:

A type of managed care plan that is similar to an HMO in that you have a clearly defined network of care. You must stay within this network to access your primary health needs. Like a PPO, this type of managed care plan does not typically require a referral to see a specialist.
